Covid Compliance Reopening Guidelines

1. I am allowing one parent to come in with each child. Parents will be required to wear a mask. They will also assist in hand sanitizing/washing when entering.
2. Temperature checks will be given to parent and child as appropriate.
3. Parents will answer questionnaires regarding COVID compliance concerns.
3. Therapy appointments will be 40 minutes as opposed to 45-50 minutes to allow enough time to sanitize in between clients.
4. Parents & siblings are to take preventative measures to minimize the spread of COVID-19.
5. Children over two being able to tolerate a face shield and/or mask will be suggested and assessed as appropriate.
Speech Time Therapy has put preventative measures in place due to the spread of COVID-19. By consenting to treatment today and in any future sessions, you voluntarily assume all risks and agree that you will not hold Speech Time Therapy or any of its employees liable for any resulting illness or injury.
Signing this document indicates that you understand the risks and agree to abide by these rules to protect yourself and protect Speech Time Therapy. Please note that if you cannot abide by these rules, you will not be able to be seen directly in my office, but can be seen online through my telehealth program. Thank you for your cooperation.
Lastly, participation in attending in person sessions or Telehealth sessions is up to the parent and/or therapist. It is not a requirement to resume in person sessions and I am happy to continue to provide Telehealth therapy for families who do not feel comfortable resuming sessions.
